Home care is often a major component of keeping your loved one safe and independent, however, increasingly, in-home care is a component of an integrated plan of care. Consulting with a Geriatric Care Manager is often beneficial, as their services can be “need specific”. The GCM can also address: medical; social; emotional and safety issues; as well as make referrals to community resources; and if needed, help with transitional housing options. A GCM can cut through the maze of elder care options and provide “peace of mind” solutions.
